CA9 (carbonic anhydrase 9) is a transmembrane protein and one of only two known tumor associated carbonic anhydrase isoenzymes. Carbonic anhydrase (CA) is a large class of zinc metalloenzymes that catalyze the reversible hydration reaction of carbon dioxide. They participate in various biological processes, including respiration, calcification, acid-base balance, bone resorption, and the formation of aqueous humor, cerebrospinal fluid, saliva, and gastric acid. Their related pathways include cellular responses to stimuli and the self degradation of E3 ubiquitin ligase COP1, an important homolog of which is CA14.

Product Background

Carbonic Anhydrase IX (CA9/CA IX) is an enzyme encoded by the human CA9 gene. It is a transmembrane dimeric metalloenzyme with an extracellular active site that promotes gastrointestinal acid secretion. CA IX is a transmembrane glycoprotein with an extracellular active site. The cytoplasmic tail of the enzyme contains three potentially phosphorylated residues (Thr-443, Ser-448, and Tyr-449) and participates in signal transduction. The enzyme exists in two almost identical dimeric forms, both of which are stabilized by two hydrogen bonds and many van der Waals forces.

Product Description

Girentuximab (trade name Rencarex) is a chimeric IgG1 monoclonal antibody to carbonic anhydrase IX (CAIX). CAIX is expressed on the surface of most renal cancer cells and is hypothesized to be on the surface of other tumor cells. It is investigational agent in clinical trials for renal cell carcinoma. Its development was suspended as a "naked" or unconjugated antibody during phase III trials due to efficacy.

Mechanism of Action

Mechanism of action of girentuximab is antibody dependent cellular cytotoxicity (ADCC). As a monotherapy in metastatic renal cell carcinomas(RCC), girentuximab demonstrated minimal clinical activity with a 3% partial response rate and a median survival of 16 months. In vitro data suggest that the number of activated ADCC effector cells can be upregulated by low dose interleukin 2 (IL2) administered in a pulsatile fashion.
