In the rapidly evolving landscape of biotherapeutics, single-domain antibodies (sdAbs)—commonly referred to as nanobodies or VHH domains—have emerged as a revolutionary class of antigen-binding fragments. Derived from the heavy-chain-only antibodies naturally occurring in camelids (llamas, alpacas, camels), nanobodies possess a unique molecular architecture. At approximately 12–15 kDa, they are only a tenth of the size of conventional IgG antibodies (150 kDa). This miniature size, combined with high physicochemical stability, enables nanobodies to access hidden or cryptic epitopes, such as enzyme active sites or GPCR clefts, that are physically inaccessible to bulkier monoclonal antibodies (mAbs).
However, the clinical translation of camelid-derived nanobodies presents a significant hurdle: immunogenicity. When introduced into the human body, non-human protein sequences can trigger anti-drug antibody (ADA) responses, neutralizing the therapeutic effect and posing systemic risks to patients. Consequently, nanobody humanization has become an indispensable phase of therapeutic discovery. By carefully mutating framework residues to match human VH sequences (specifically human VH family III) without disrupting the CDR loops responsible for antigen binding, researchers can generate molecules that are virtually indistinguishable from human proteins by the immune system, while fully retaining their target affinity and exceptional stability.
